Good Glucose Launch Giveaway – Official Rules

1. Sponsor

DMS Natural Health Inc, 810 Busse Hwy, Park Ridge IL 60068 ("Sponsor").

2. Eligibility

Open only to legal residents of the 50 United States and D.C. who are 18 years of age or older (19 in Alabama and Nebraska; 21 in Mississippi) at the time of entry. Employees of Sponsor, its affiliates and agencies, and their immediate family and household members are not eligible.

3. Entry Period

Begins 12:00:00 a.m. CT on October 19, 2026 and ends 11:59:59 p.m. CT on October 30, 2026. Sponsor's clock is the official timekeeper.

4. How to Enter

Visit justthrivehealth.com/pages/good-glucose and submit a valid email address through the entry form to opt in to Sponsor's marketing emails. Limit one (1) entry per person and per email address. Entries made with multiple or fake email addresses, scripts, bots, or other automated means are void. Sponsor is not responsible for lost, late, incomplete, or misdirected entries, or for any technical failure that prevents an entry from being received. You may unsubscribe from marketing emails at any time. Unsubscribing after entering does not affect an entry already submitted.

5. Prizes

Five (5) winners will each receive:

  • Three (3) bottles of Good Glucose (ARV $237.00)
  • One (1) 90-Day Probiotic (ARV $129.99)
  • Three (3) bottles of Digestive Bitters (ARV $89.97)

Total ARV per prize is $456.96. Total ARV of all prizes is $2,284.80. Prizes are not transferable. There is no cash equivalent or substitution, except that Sponsor may substitute a prize of equal or greater value. Prizes ship free, to U.S. addresses only.

6. Drawing and Odds

Winners will be selected at random from all eligible entries on or about November 3, 2026. Odds of winning depend on the number of eligible entries received.

7. Winner Notification

Winners will be notified by email within 2 business days of the drawing. A winner must reply with a U.S. shipping address within 72 hours. If a winner doesn't respond in time, the notification is returned as undeliverable, or the winner is found ineligible, that prize is forfeited and Sponsor may select an alternate winner.

8. Taxes

Any taxes on a prize are the sole responsibility of the winner.

9. Publicity

Except where prohibited, accepting a prize grants Sponsor permission to use the winner's first name, last initial, and city/state for promotional purposes without further compensation.

10. Release

By entering, entrants release Sponsor, its affiliates, and Meta (Facebook/Instagram), TikTok, and any other social platform used to promote the giveaway from any liability arising from participation in the giveaway or from accepting or using any prize. Consult your healthcare provider before starting any dietary supplement.

11. General

Sponsor may cancel, suspend, or modify the giveaway if fraud, technical failure, or any other cause beyond its control affects its integrity. Sponsor may disqualify anyone who tampers with the entry process or violates these rules. Sponsor's decisions are final.

12. Governing Law

These rules are governed by the laws of the State of Illinois. Any dispute arising from the giveaway will be resolved individually, without resort to any class action, in the state or federal courts located in Cook County, Illinois.

13. Privacy

Personal information is collected and used in accordance with Sponsor's Privacy Policy at https://privacy.justthrivehealth.com/.

14. Winners List

For the names of winners, email support@justthrivehealth.com or send a self-addressed stamped envelope to the Sponsor address above by December 31, 2026.
